The most common form of sleep apnea is obstructive sleep apnea (OSA).

Diagnosis and severity are typically determined through an overnight polysomnography or Sleep Study, that will give the results of the Apnea Index and the Apnea/Hypopnea Index, as well as the oxygen saturation.

I have several carriers that will issue at Standard ratings.
